I understand that to complete my registration and save my spot in the class(es) I have chosen I need to send in my dancer's first month (September) tuition to [email protected] *Please include dancer's first and last name in the "memo" of the e-transfer - this fee is 50% refundable before August 15. Passed this date, it is 25% refundable. Once classes begin on September 7th it is non-refundable.
My dancer is taking 1 class, I will send in $65 plus HST for a total of $73.45
My dancer is taking 2 classes, I will send in an additional $55 plus HST($63.15) for a total $135.60
My dancer is taking 3 classes, I will send in an additional $55 plus HST($63.15) for a total $198.75
I am aware that my monthly tuition is due on the 1st of each month. I will send my first month's tuition upon completing this form in order to secure my dancer's registration. I acknowledge that my next payment is due October 1st.

I am aware that my dancer will participate in a Year-End Recital sometime in June and I need to purchase a costume for this performance. *Please note that for dancers enrolled in more than one class, they will need to purchase a separate costume for each class that they are in. Each costume is $120 plus HST and is due November 1st

My dancer is enrolled in a 16-week Sunday class and will not participate in the Year-End Recital
Dancer's in classes Primary and above are required to purchase a TDM T-Shirt to wear to their class(es) each week. Please select the size you wish to order. Payment for T-shirts will be due upon receipt of the shirt in September:
Child extra small (typically fits Primary aged dancers)
Child small (typically fits Junior aged dancers)
Child medium (typically fits Intermediate aged dancers)
Child large (typically fits taller Intermediate aged dancers)

I do not need at T-shirt, mine still fits from last year
I have a mini aged dancer and do not need a T-Shirt. Female Mini dancers wear pink bodysuit, pink ballet tights and pink ballet slippers to their classes. Male Mini dancers wear black shorts and a white t-shirt with black dance slippers. TDM does not sell these items. We recommend purchasing these from Amazon or Dancewear Plus here in London.
